Bronx Immigration Medical Exams / Physicals

In order to apply for a green card, citizenship, or permanent residency, you will need to have an immigration medical examination. The purpose of the physical is to determine whether, in the interest of public health, it is safe for you to enter the United States.

This physical exam must be performed by USCIS certified civil surgeons, who are specially trained in issues concerning immigration. At Boulevard Medical Healthcare in the Bronx, our USCIS certified civil surgeon (USCIS doctor) Dr. Nauka Desai will perform the medical exam.

If you are applying to have this immigration physical exam performed outside the United States, the consulate should provide you with a list of qualified physicians before your visa interview. Otherwise, you can find a list of immigration medical exam locations on the state department website.

What To Bring To Your Immigration Physical Exam:

  • A copy of immigration form i-693. Please fill out your portion before coming to your physical.
  • A list of your prior vaccinations with proof.
  • A condition report and a list of supervision or education requirements for anyone in your family who suffers from mental or learning disabilities.
  • If you have ever been diagnosed with TB, you must present a written certification indicating any treatment and how long it lasted. It must be signed by your doctor. The certificate must include dates and types of medications you took. If you have ever had an abnormal chest X-ray, bring them to the exam. The actual films, not the typed reports, may be required.
  • If you have had syphilis, a written certificate, signed by a doctor or public health official must be presented to your USCIS doctor. If you have ever had a positive blood test for syphilis and were not treated, you must bring an explanation in writing from your doctor.
  • If you have any history of harmful or violent behavior resulting in injury to people or animals, or damage to property, you will need to provide information that explains why. Harmful behavior includes attempted suicide or self-harm, no matter how minor in nature.
  • If you have been treated or hospitalized for psychiatric or mental illness, or alcohol or drug abuse, you must have written certification including the reason why, the length of treatment, and your treatment plan.
  • Be ready to explain any chronic medical condition(s) to your USCIS doctor and give the names of the medications.

What the Immigration Medical Examination Consists Of:

  • Discussion of your medical and vaccination history
  • Your immigration doctor will administer any missing vaccinations or tell you where to get them
  • Blood test.
  • Chest X-ray (may be postponed due to pregnancy, speak to your consulate)
  • Physical exam (eyes, ears, nose, throat, extremities, heart, lungs, abdomen, lymph nodes, skin and external genitalia)

After the physical exam, your USCIS certified civil surgeon will complete the rest of the I-693 form and present it in a sealed envelope for you to submit to the Department Of Homeland Security and U.S. Citizen and Immigration Services along with the rest of your citizenship, green card, or permanent residency application. Do not open the envelope, as the USCIS will not accept envelopes that have been altered.
